 RN Case Manager (Full-Time)

HoriSun Hospice is seeking a compassionate, qualified Full-Time RN Case Manager to serve hospice patients in the Lincoln and western service areas, including Seward, Milford and Crete. Responsible for providing nursing care and coordinates the implementation of the plan of care for each patient, and case management to hospice patients and families. Director of Clinical Services

Reports to: Executive Director


General Purpose:

Responsible for planning, developing, implementing and evaluating all patient/family services. Manages patient care staff. Supports the Operational and Provider Relations activities of the agency.

Essential Job Functions:


Compliance      Duties: Ensures that all patient and family services offered by the agency  comply with federal, state and local regulatory requirements according to the standards established by the State Board of Nursing.


Clinical Operations  Duties: Develops, implements and evaluates clinical operations policieand procedures and a system of staff communication that ensures coordinated implementation of plans of treatment and the utilization of necessary services for comprehensive care of the patients that reflects the philosophy and objectives of HoriSun Hospice.


Management              Duties: Ensures staffing/coverage to provide sufficient personnel for delivery of patient/family services, including on-call, pursuant to agency policies and guidelines. Makes recommendations for staffing to Executive Director.


Staff Development     Duties: Plans, implements and documents the orientation and ongoing education of the clinical staff. Participates in the orientation and ongoing education of the clinical staff in the hospitals where hospice has contracted for in-patient hospice beds. Participates in the development of personnel policies and position descriptions that delineate qualifications, duties and responsibilities for all levels of clinical staff.


Quality Assurance     Duties: Maintains ongoing quality assurance activities related to patient and family services including quality improvement (QI) studies and chart audits to ensure the quality of patient/family services.


Human Resources     Duties: Responsible for all aspects of  Human Resources. In collaboration with the Executive Director, manages employees by establishing annual performance expectations, assessing performance, correcting performance, and recommending individual compensation increases to the Executive Director. Ensures appropriate recruiting, interviewing and hiring of qualified staff. Takes appropriate measures to manage poor performance and quickly address violations of agency policand procedureFamiliarizes all associates, as well as contractual providers with the philosophy, goals, policies and procedures of the agencyPlans and provides orientation, in-service and continuing education programs to promote professionalism and personal growth of associates. Maintains appropriate personnel records,  administrative records and all policies and procedures in the hospice locations.


Financial                    Duties: Participates in the budgeting for patient-care expenses and

Management              monitors expenses related to patient/family services. Collaborates with

Executive Director to maintain salary/wage/benefit goals.


Liaison                       Duties: Serves as aresource person and liaison for the hospice care team members who do not directly report to Director of Clinical Services. Leads the interdisciplinargroup meetings and serves as a liaison with the medical director and clinical staffServes as a liaison with vendors and providers affiliated with hospice on a contractual basis. Monitors the quality of services provided by these vendors/agencies.


Community                Duties: Participates in state and national hospice organization activities.



Other                          Duties: Contributes to team effort by accomplishing related results as needed.


Physical & Sensory Requirements (with/without thaide of mechanical devices): Mobility, reaching, bending, lifting, grasping, fingering, fine hand coordination, ability to talk and hear; ability to read and write; ability to communicate with personnel, ability to remain calm under stress.



•           Must be a registered nurse and maintain current Nebraska State license.

•           Must meet all state and federal regulatory requirements.

•           CPR certified

•           Must have at least threyears clinical management experience.

•           Must have ability to direct and supervise clinical staff.

•           Previous hospice management experience beneficial.

•           Knowledge of hospice reimbursement requirements preferred.

•           Must have the ability to foster teamwork and lead in a professional manner.

•           Must be able to contribute as a senior level business leader of the agency.

•           Must be able to manage patient care in a caring, cost-effective manner.

•           Must have experience in providing services to terminally ill patients.

•           Must demonstrate a commitment to the hospice philosophy of care.

•           Must be able to relate professionally and positively to management staff and to work cooperatively with other associates at all levels.
