Clinical Records Quality Assurance Specialist

The primary duties for the Clinical Records Quality Assurance Specialist are medical records and quality assurance/utilization review, survey preparation, intake assistance, and serving as a resource for the clinical staff around charting and billing. The Clinical Records Quality Assurance Specialist also provides general support for all programs and other staff as needed. Provides clerical and telephone support to the Interdisciplinary Team (IDT) members and other personnel as requested. Responsible for assuring medical records are complete and that they comply with agency policies and state and federal regulations. Assists with special projects as requested. Maintain organization of medical records and office supplies. Other duties as assigned.

The Clinical Records Quality Assurance Specialist will be evaluated on the adequacy, appropriateness and timeliness of the following criteria:

Clinical Services

  • For all new hospice patients, sets up binders for the medical record.

  • Files and organizes documentation in the medical record.

  • Monitors medical records for completeness of required forms, signatures and correct dates.

  • Obtains all needed demographics, H&P, records to initiate patient record.

  • Completes and faxes admitting orders, POC and IDT to attending MD and Medical Director.

  • Calls for and obtains telephone orders from physicians when necessary.

  • Monitors unsigned physician orders and plans of care.  Submits follow-up requests for signed orders to overdue physicians.

  • Files medical record forms.

  • Provide detail-focused review of clinical documentation to ensure quality, regulatory compliance and audit-readiness.

  • Faxes documents as requested by staff.

  • Revises forms as necessary and discards outdated ones.

  • Completes medical records (including record audit) within 30 days of discharge, death or revoke.

  • Protects all clinical records through security procedures.

  • Coordinates requests for chart copies from outside agencies/persons, using HIPAA guidelines and direction from DOCS.

  • Files all completed Medical Records alphabetically in locked files.

  • Compiles information and Admission packets or delegates to office volunteers.

  • Manages clerks/volunteers who assist with clerical duties within medical records.

  • Prepare charts for patient transfers.

  • Serves as a clinical resource to patients, family and interdisciplinary team.

  • Leads the Quality Assurance/Quality Improvement committee. Assists with chart audits and projects.

  • Ensures patients’ and families’ right to confidentiality of medical and personal information.

  • Ensures EMR records are accurate and complete which enables billing to be done in a timely process.

  • Provide ongoing training and support to clinicians for documentation, charting, and billing procedures.

  • Provide administrative assistance to the clinical intake process.

  • General administrative support such as answering and directing calls, assisting clients, and other support tasks.

  • HIS/CAHPS/CMS/QAPI.

 

Interdisciplinary Team Collaboration and Communication

  • Notifies IDT members when documentation needs to be completed.

  • Works in a manner that helps foster teamwork and problem solving.

  • Conforms to scheduled work hours with punctuality and regular attendance, notifying supervisor in advance of planned absences.

 

Education of Patients, Families, Community and Staff Members

  • Assists with mailing of Information Packets to prospective patients/families and health care providers.

  • Attends educational events as available and requested to do so.

 

Quality Assurance/Quality Improvement

  • Assists with QA/QI committee with chart audits and QA/QI studies.

  • Participates as a member of the QA/QI Committee and QA/QI meetings on a regular basis.

 

Professional and Personal Growth

  • Identifies personal educational needs and attends in-service training programs to meet these needs.

  • Participates in self-evaluation to identify areas where improvement is needed and develops a plan to accomplish improvement.

  • Develops annual goals and a plan to accomplish goals.

 

Qualifications:

  • Must be a licensed practical nurse or registered nurse with a current Nebraska State license.

  • Must have current certification for CPR.

  • Must have a minimum of 1-year general nursing experience.

  • Must demonstrate a commitment to the hospice philosophy of care.

  • Must be knowledgeable of Medicare hospice certification.

  • Must have experience in providing services to terminally ill patients.

  • Must have a valid drivers license.

  • Must demonstrate excellent observation, nursing judgement and communication skills.

  • Must possess the ability to provide good written documentation in a timely manner.

  • Must be self-directed with the ability to work with minimal supervision.

  • Must be capable of maintaining regular attendance.

  • Must be able to relate professionally and positively to management staff and to work cooperatively with other associates at all levels.

  • Must meet all local health regulations, pass post-offer drug test and pass post employment physical exam if required.

  • Must be capable of performing the essential job functions of this job, with or without reasonable accommodation.

  • Previous experience with an electronic medical records program, billing, and audits preferred.

  • Previous hospice or quality assurance clinical records experience preferred.

Hospice Aide (Full-Time)

Responsibilities Include:

  • Work under the supervision of a registered nurse and perform various services for a patient as necessary to meet the patient's personal needs and to promote comfort
  • Responsible for observing the patient, reporting these observations, and care performed
  • Promote quality, continuity, and safety of a patient's care
  • Must have a current CNA certification

Hospice Social Worker

HoriSun Hospice is seeking a Full-Time Masters Social Worker (MSW). The Masters Social Worker is a qualified professional who, in accordance with the plan of treatment, assists the physician and other team members in understanding the significant social and emotional factors related to health status. This individual participates in the development of the plan of treatment, prepares clinical and progress notes, works with family members, utilizes appropriate community resources, participates in discharge planning and inservice programs, and acts as a consultant to other agency personnel.

Education, training, experience:

  • Master degree in social work
  • One (1) year experience in social work in a health care setting
  • Current driver's license and reliable transportation

 RN Case Manager (Full-Time)

HoriSun Hospice is seeking a compassionate, qualified Full-Time RN Case Manager to serve hospice patients in the Lincoln and western service areas, including Seward, Milford and Crete. Responsible for providing nursing care and coordinates the implementation of the plan of care for each patient, and case management to hospice patients and families.
