Tracey McChargue, LCSW, APHSW-C
Hospice Social Worker
Tracey received her Master of Social Work from the University of Illinois – Chicago with an emphasis in Mental Health. She received her Bachelor of Science in Family Social Services and Sociology from Northern Illinois University. Tracey has worked for 15 years in medical social work in the skilled nursing community, hospital and dialysis settings. Previously, Tracey worked in child welfare as well as program development and training/education. She has enjoyed working with individuals and families through the lifespan; from before birth, preparing and educating families for adoption and helping them adjust to parenthood, to providing end of life support to patients and families through their journey on hospice.
